Here's how you can claim your**FREE** ice cream: First of all, you will need to visit Mayfield Ice Cream's website. Once you are on their homepage, you will see an icon to download their rebate form. Click and print.
Also on that same web page, you will see an icon to print out a coupon good for $1/1 Mayfield ice cream (1.5 quart carton or larger only). The rebate form assumes you used the $1 coupon, so make sure you print out the coupon and use it when you go!
